About chronic disease management
How often will I need to be seen?
It depends on how controlled your condition is. Typically every three to six months once things are stable, and more often while we're adjusting treatment. Telemedicine visits can cover many check-ins.
Can you take over management from another doctor?
Yes. Bring or send your records — we'll help request them — and we'll continue or refine your current plan rather than starting from scratch.
Do you manage medications and refills?
Yes. Medication management is central to chronic care: we review everything you take, simplify where possible, and keep refills running on time.
